The enrollment process can be completed entirely in your own home by applying online.
If you need help with your online application or if you want to complete an application over the phone, call and speak to a Health Benefits Coordinator (HBC) at 1-800-701-0710 (TTY: 711).
If you would like to have face-to-face contact with someone, you may select one of the many outreach and enrollment sites listed by county below.
Due to the COVID-19 crisis, some offices may not be open. Please call before visiting.
